Assessing the efficacy of value-based care in mitigating rising healthcare costs: A case study

dc.description.abstractThe South African healthcare system, characterised by a stark dual-tiered structure, faces unsustainable cost escalation, with private sector premiums rising 9-10% annually and nearly half of the country's 8.5% GDP health expenditure consumed by only 16% of the population. The prevailing fee-for-service model incentivises volume over value, exacerbating inefficiencies and financial strain. Value-Based Care (VBC) has emerged as a transformative model in high-income countries to address such challenges. However, its efficacy in low- to middle-income countries (LMICs), such as South Africa, remains underexplored. This study assesses the effectiveness of VBC in mitigating rising healthcare costs within this unique context, using a qualitative case study of PPO Serve's partnership with a leading medical aid group. Data was collected through semi-structured interviews with 12 healthcare professionals and administrative staff at PPO Serve and analysed using thematic analysis. The findings indicate that VBC implementation, through integrated care coordination and preventative strategies, led to improved patient outcomes, including a reported 15% reduction in hospital readmissions and enhanced patient satisfaction. Furthermore, the model demonstrated effectiveness in reducing avoidable healthcare utilisation, thereby contributing to cost savings of 7-12% and better financial predictability. However, significant implementation challenges were identified, including data infrastructure fragmentation, financial incentives misaligned with VBC goals, and internal resistance to organisational change. The study concludes that VBC is a viable strategy for reducing healthcare costs without compromising care quality in South Africa. It recommends strategic interventions such as strengthening health information systems, realigning reimbursement models with value-based incentives, and investing in comprehensive stakeholder engagement. This research provides critical empirical evidence on the adaptation and application of VBC in seeking to achieve sustainable and equitable healthcare delivery in resource-constrained environments.
